Flu Vaccines: The Health Dept. will be at May ISD on 10/8/19 for flu vaccines. If your child has medicaid, it will cover the vaccine. Send back the form with a copy of the card. If he/she has Chips insurance, send a copy of the card with the co-pay amount. If the child does not have insurance, the cost will be $14.00 and with insurance the cost will be $25.00.
United Pharmacy will be here on 10/9/19. If the student has insurance, they will file the insurance. The student has to be at least 7 years old. If parents or community members would like a flu vaccine, they are available on both days from 8-10 am.
JH and HS student can pick up a vaccine form in the HS office, and forms were sent home with all elementary students in their green folders.
